Promise Healthcare receives Patient-Centered Medical Home recognition

CHAMPAIGN, IL (Chambana Today) — On Friday, Promise Healthcare announced that it received Patient-Centered Medical Home (PCMH) recognition from the National Committee for Quality Assurance (NCQA). The recognition reflects the organization’s commitment to delivering high-quality, coordinated, and patient-centered care to its community.

Promise Healthcare is a federally qualified health center which provides affordable healthcare regardless of a patient’s ability to pay. It has four locations in the C-U and one in Rantoul, which can be seen below.

The NCQA’s PCMH recognition program is a nationally recognized distinction that validates primary care practices for their focus on quality improvement, care coordination, enhanced access, and team-based care. Practices that earn this recognition demonstrate the ability to provide comprehensive, patient-focused care that improves outcomes and strengthens the patient experience.
